Vaginal Discharge

Hi. I am a 20 year old female. I have a daughter (aged 1year and 5 months) I stopped breast feeding her at 9 months. I am having unusual periods the past 4 months. I use to have a period 4-5 days now it is 7-8 days long. This month has been very light and has been on and off. I will have it 1 day the next I won't and so on. On day 5 of my period I had very sharp pains in the pubic area. (Was nothing like period pain) I use tampons and change them often. I am not on any contraceptive. I have noticed the past few days I have a discharge, clear and a white thick discharge with an odor. Please help?

Thank you for your question. Often we find the act of breast feeding will change the menstrual cycle, acting as a form of natural contraceptive. But in your case more than 9 months has passed between the cessation of breast feeding and the onset of your current symptoms so I don't think there is any relationship.

What you describe sounds most likely like a vaginal infection, most likely candidiasis. I would advise seeking professional help to confirm diagnosis and formulate both pharmaceutical and natural otions for treatment. Good day Dr i have been on the petogen contraceptive since dec'06, i stopped the contracertive last year. my last injection was in sept 2010. i started having my periods in april this year. the first month was a normal 5 day cycle, then in May - it lasted 11 days, June - it lasted about 3 days, and this month - 3days. but it was early cos i had my period at the beginning of june and at the end. now i am experiencing a very strange discharge and i'm not sure what it is. the last time that i was sexually active was about a month ago, without protection. the discharge is thick, sort of slimy sticky and transparent with the odd bits of white in it. what could be the possible reasons for this?

Thank you for your question. It often takes up to a year for your body to get back to a normal regular cycle after the complete cessation of all forms of synthetic contraceptives. As such I think it is too early to draw any conclusions from the changes in the onset/duration of your cycle.

However the discharge you describe is not normal. From what you describe it sounds more like a candida infection more than any other form of vaginal discharge. You can confirm this by testing via a culture of the fluid. Any GP will be able to arrange this.

If it is indeed candida you have two options available:

1. A pharmaceutical approach that is supposed to kill the fungus (although today we are seeing a lot of drug resistance so I would ask the GP doing the culture to ask for a drug sensitivity before you begin the drug). 
2. A wholistic approach using plant-based compounds that are as effective as the pharmaceutical but without the side effects. Examples here include products including grapefruit seed extract, berberine and garlic extracts. But as above I would wait to confirm the candida diagnosis before beginning treatment.

With either of these options I would also recommend high doses of a fridge-stored probiotic, at least 10 billion organisms per day. 

I would also recommend looking at the rest of your body to look for symptoms of candida presence. Usually the vaginal discharge is the tip of the iceberg. If there are other symptoms then consider a good cleanse or detox program combined with immune-boosting supplements to enhance your immune system. Candida is a normal part of our flora but only becomes problematic when the immune system is compromised and the balance between healthy flora and unhealthy fungi like candida changes.

I sincerely apologise for the delay.  Some of us have been on sabatical.

It would be a good decision to have a rest from the 3 month (12 week) contraceptive injection, as you have been on it for almost 3 years (you do not give exact dates).  You should consider detoxifying for at least 6 months due to the length of time you were taking Depo-Provera, or perhaps you were on the generic alternative which is Petogen.  The same applies.

It is normal not to experience a period when on the contraceptive injection after a lengthly time span.  However, the 'funny smell' you describe should be checked out.  I suggest that you have an annual cervical smear (pap smear) so as to diagnose any possible infection in the vaginal area.  An appointment can be made with your gynaecologist, or alternatively with our Kloof Clinic in Cape Town for a cervical smear to be done.

Sorry,the other thing is I have this yellowish spotting on my pantyliner, for a time now. And sometimes after intercourse I have a brownish spotting... is it something I need to worry about? In my previous question to you I forgot to say that in the past month I took 3 home pregnancy tests, but all showed negative. I am really concerned.

Thank you for your new question.  Your signs and symptoms indicate that you need to have a Pap smear (cervical smear) done so as to rule out a vaginal infection.  I suggest that you have a urinalysis done as well,  so to check for a urinary tract infection.

I suggest that you contact your medical doctor for an appointment as from the description given of the odor you will probably require a pharmaceutical antibiotic due to a possible vaginal and or urinary tract infection.  The sooner this is resolved for you the better for your health.

Thank you for your query.  I suggest that you make an appointment to see your medical doctor as I suspect an infection.  Your doctor may suggest doing diagnostic tests, inclusive of a cervical smear.  Depending on your results, an antibiotic may be prescribed.  The causes could be one of an assortment of reasons, e.g. a vaginal infection, bad eating habits, not drinking sufficient water, even using a harsh soap with a high pH level.

I am suffering from a white smelly discharge. My gynae told me he doesn't have to give strong medication as I'm in my early months (11 weeks pregnant). He prescribed 1 gyno-pevaryl depo and forcid solutab, this is the second time I am using gyno-pevaryl depo, after 4 weeks ago it did not help only a day or two after taking from there that problem started again. This worries me because I'm afraid to have sex with my man.

Your doctor is treating you for a suspected Candida infection. This can be more prevalent in pregnancy and can be one of the reasons for the recurrence. It could also be that you have a stain that is more resistant. The other cause could also be reinfection by your partner.As the treatment has not worked a second time now, I would recommend you visit your doctor again. From the sounds of the medication that you have described it sounds as though your doctor may be treating a Candida yeast infection (otherwise known as thrush). This normally responds well to this treatment, but you will need to finish the course of treatment. In the meantime, I would recommend that you abstain from intercourse. There is also the possibility that your partner may also have the infection and not know about it. In cases where regular re-infection occurs it may be necessary for both you and your partner to take oral medication. I would recommend that you finish the course of treatment and if it has still not passes that you visit your gynae or GP again. Regards Dr Sean Gomes

I just wanted to confirm that this is healthy. Don't recall having it this obvious before. I have a milky white (snotty kinda) discharge. It has no obvious smell. Although its a lot at times and isn't itchy or burning or sore or nothing. Just not sure if I should go see my gynea. I am using gynea guard to cleanse my personal area, as I did have an odour so that seems to have cleared it. But now I see a white white discharge. I'm no longer using soaps. I wear cotton underwear. I am on the Yasmin pill for over 6 months now and am 26 years old. I'm not sure what other information you require. Please let me know what is healthy and what isn't.

It is not uncommon for women to often have some form of discharge. This can either be pathological (infective) or physiological (related to normal hormonal cycle). This can be associated with ovulation. Typically, odorous discharge is indicative of a infection or an imbalance in the natural bacteria that inhabit the vagina. An infection could either be bacterial or fungal. You are practising the correct measures by wearing cotton underwear and no longer using soaps. As you have not changed your contraception recently, I would recommend that you do visit your doctor for a check-up. They will be able to do some simple swabs which will help exclude any form of infection that may need treatment.

I was on Clomid this month for the first time (50g) and normally I see my periods on the 28. Since the 20th I have been getting abdominal pains and mucus like discharge. What could be the problem?

The Clomid tablet is a fertility drug prescribed by a medical doctor for a patient who has had a problem conceiving and would like to do so.  Abdominal discomfort is a side effect of taking Clomid.  What does your mucus discharge look like, i.e. colour, consistency and odour?  Should the mucus have an unpleasant odour you would need to contact your doctor.  Your hormonal cycle will be upset as a result of the increase of hormones in your systems due to taking this fertility medication.  I suggest you contact your doctor again should you continue to be concerned.  Kind regards, Angela, Nursing Practitioner.

I also have a vaginal discharge , but it is NOT milky , not thick . My underwear gets very wet , it is almost like urine and after a while starts to smell a lot like ammonia. It is so strong I can smell it , I am to scared to sit near people, or open my legs even a little because of the smell. If I don't change my underwear , it can begin to smell , almost like dead rat , it is terrible and very embarrassing. Is this a discharge problem or a bladder problem. I at first thought , bladder problem , but why would urine start to smell so awful once starting to dry on the underwear ?? Please help me.

This does seem to be a possible vaginal infection. I suggest that you definitely make an appointment to visit one of our Well Clinics. Two procedural diagnostic tests should be done for you with the signs and symptoms you are experiencing. A pap smear will determine a vaginal infection and the probable organism causing the infection. Then the correct antibiotic and any other medication required can be prescribed for you where necessary. A direct referral can be made to a medical doctor/specialist should a heavily scheduled antibiotic be recommended by a pathology laboratory, once the pap smear results have been received by our clinic. I'm a bit confused with my body. I've been on Triphasil for almost 8-9 years. In 2003 I had my first baby and after the birth they injected me with Nur-isterate. But when I went to my next appointment after the first 2/3 months, I asked the nurse for Triphasil because I was used to it they gave it me and I stayed on it until I had my second baby 25/01/2009 and this time they gave me Petogin injection which after a month I bled like anything I don't know whether I was getting my periods or what was happening but eventually it stopped after almost two weeks. I went for my first appointment after and I was given a shot at the clinic after birth same thing happened but this time I bled for 4 weeks non stop then I went to the doc and he explained to me something about my hormones which I did not understand and he gave the Nordette pill which he said will stop the bleeding but I will still be safe with using that but now I'm experiencing a lot of wetness and a white thick sticky discharge which is not smelly or itchy but I'm constantly wet from it and I constantly need to go and relieve myself . So my concern was can it be the Nordette Pills or do I have an infection. This discharge is really bothering me because it's affecting my sex life. I don't need stimulation because I'm already wet from the discharge. Please I need your help urgently!

First of all, the Nordette contraceptive pill will not have caused your ‘white thick sticky discharge’, nor the constant sensation of being ‘wet’. You most definitely do need to visit one of our Well Clinics for a consultation and have a urinalysis done from a urine specimen, and at the same appointment request a pap smear. Please ask for a time allocation of an hour for this appointment. The urine specimen can be tested in the clinic to determine if there is a urine infection which could be the cause of you urinary frequency. The pap smear would be sent to a pathology laboratory for analysis to determine any infection and cell changes of the cervix. These procedures are both diagnostic and would determine the extent of your problem and if medication is required to relieve the above mentioned signs and symptoms. You should be able to continue with Nordette, and I suggest that you keep taking it in the meantime. Alternatively, you can visit your general practitioner or gynaecologist. The use of Vagifem could predispose you to a yeast infection such as Candida. This can be treated with an antifungal such as Canisten. A fish like odour cold also be typical of a bacterial infection such as Bacterial Vaginosis. This would require an antibiotic.
