Thank you for your question. I would strongly suggest that your husband go visit his GP or local clinic. It sounds like he's got a respiratory tract infection, and from the other symptoms I suspect it may be tuberculosis. This is a disease endemic to the Western Cape, and if his immune system was under duress from other factors, such as poor diet, insufficient activity levels, excessive emotional or psychological stress, or other medical illnesses, it may have opened the door to this opportunistic infection.
It's imperative he have a sputum test performed as a simple chest x-ray is insufficient to make a definitive diagnosis. Even if your doctor insists on a trail of antibiotics I would still ask for the sputum test. Things he could do to support his immune system include increasing his Vitamin C intake (in an esterified form), supplementing with Vitamin D3 and perhaps olive leaf extract.
